High Risk Outreach
Each year, the Western Regional Addiction Resource Collaborative, through funding from NY State OASAS, is looking to support high-risk outreach efforts to local communities in need in the five target counties (Erie, Niagara, Allegany, Chautauqua, and Cattaraugus).
What we are looking to support:
1. Community awareness type projects that are found within high-risk regions across the five counties.
2. All high-risk outreach projects will need a data-based justification as to why the particular target population is high-risk.
3. Projects that cost between $1,500 and $3,000.
What we will not support:
1. Projects that offset costs of already existing programs/services.
2. Projects that do not contain some type of outreach.
3. Projects that cannot produce some level of data justifying the target population as high-risk.
If you have a project that fits these criteria, please complete the request form found below. At a future CAP meeting, your request will be reviewed and you will be contacted regarding the decision to support your project. If accepted, you will be required to send an invoice for the accepted support amount, as well as a W-9 form for your agency (found directly below).

If your project is supported, you will be required to complete a final report (found below). An additional requirement is to create some sort of media noting that the event was sponsored by the "Western Regional Addiction Resource Collaborative" and containing the following logo:

The media can be a direct mailer, an e-mail to interested parties, an announcement, a flyer etc. You will be required to submit at least one form of proof of performance containing the media and logo upon completion of the project. These materials can be sent to [email protected].
